[Wyewood] Aug-26 Business Meeting Outtake

Greetings everyone,
 
Populace who attended the business meeting on Aug-26 requested that a post-event
survey be implemented to provide our Event Stewards with input.  I have created
a SurveyMonkey account and started putting together just such a survey.
 
Right now it has:
1. What went well?
2. What needed improvement?
3. Contact info if you wish to be contacted (SCA Name, Email, Phone)
 
Is this enough or do you feel there should be additional questions posed?
