[Wyewood] Survey feedback

This survey is really only for those that attended the event.
An idea that occurred to me is setting up surveys along two tracks (if possible).  First question could be, did you attend or not attend the event?  Attending the event leads into one set of questions like in this survey.  Not attending leads into a different set of questions.
Feedback from those that do not attend could provide some useful information on why they did not attend and possibly be of use for planning of future events.

Greetings,

At our August business meeting I was tasked to put together a SurveyMonkey
evaluation to be used as a post-event commentary from attendees. The survey for
Wyewood Champions is ready to roll out and I will keep it open for 10 days. As
of September 11, I will gather the commentary and forward it to the Event
Steward. It will also be filed with the event report.

Going forward we will be incorporating this post-event evaluation as part of our
event process.
