[Wyewood] New info for filling out heraldric submission paperwork

A note to anyone getting ready to write up and submit a device, name, or
badge.  The decision has been made that the "date of birth" and "gender of
submitter" lines are no longer to be filled out on the forms.    
Don't worry if you just sent things in with this information, it will just
be white'ed out.
